CYC Photo ID Cards
CYC ID card requests have increased due to the new requirement which requires all players, regardless of age, to have ID cards.
Reminder, all coaches must have a CYC ID card also. You will need your Coach’s ID number which can be found on your coach’s certificate. This is obtained once you have completed the CYC Coach’s Training program.
After your card order has been approved, you will receive an email stating that your card is “in process.”
When you placed your order, the email with your receipt also had a link to your temporary waiver. Please be sure to print out the waiver. This waiver acts as a substitute for the ID card and is good for three weeks from the date you placed your order.
Our staff is working as quickly and efficiently as possible to approve, print, and mail out the CYC cards.

All coaches and players should obtain their cards prior to the start of their season. No cards applied for after September 30, 2016 will be processed until after the Soccer/Volleyball Playoffs.
For those who miss the deadline, you need to have your parish rep email our office with your information and reason as to why deadline date was missed.Your district representative should also be included on this email.
Cards are being processed. Please retain your receipt as this is used as your ID Card waiver for 3 weeks after applying for your card.
After September 30, 2016, the system will no longer produce a waiver, as it assumes all orders after this date are for a future sport.
All Coaches, both Head or Assistant, and all Managers for Kindergarten thru 8th grade must have taken the CYC 'Coaches Training' program on line training, or attended an in-person session.
After completing the training program, they will then need to apply for a CYC ID Card.
All Coaches, in all age groups must have a CYC Photo ID Card.
After verification from the Safe Environment Program, that they are in compliance with the Archdiocesan requirements, they will then be issued a CYC Coach's picture ID Card with their Coaches ID number on it.
Head Coaches/Managers for High School and Adult Divisions do not need to attend the CYC-Sports' Coaches Training Program, but they do need a CYC Photo ID Card.
All Players, Pre-K through Adult, must have a CYC Photo ID Card starting with the 2016 Soccer/Volleyball Season.
- Please do not arrive before the published starting times. Registration will not start early.
- Facilities are in use until the published starting times.
- Additional (duplicate) CYC Photo ID cards may be purchased at the time of registration.
Because this is a photo ID, the applicant must be present to have his or her picture taken. All the ID sessions are open to anyone. You will also need to bring one of the following for each applicant, for verification of birth date (the item must have the applicant's correct name and birth date on it):
- Birth Certificate
- Baptismal Certificate must have Birth Date indicated
- Current or Expired CYC Photo ID Card
- Valid Drivers' License
There is also a $5 processing fee for each ID card. Note: Fee has increased to $5 effective 9/24/09
